Allegany County Health Planning Coalition is the County’s Local Health Improvement Coalition (LHIC).

To appropriately improve the health and wellbeing of our county, we must learn from our population and seek to understand the diverse effects on our health. To enhance the lifelong health of all Allegany County residents, we engage with the two of the largest organizations impacting our community’s well-being: the Allegany County Health Department and our healthcare system, UPMC Western Maryland. This commitment is further strengthened with representatives from other organizations and members of the public, whose support and participation are greatly valued.

We actively seek diverse partnerships to effectively improve community health and well-being, understanding that health is multifaceted and influenced by wide-ranging factors.

The Local Health Action Plan outlines strategies to improve health and wellbeing in Allegany County based on the results of the Community Health Needs Assessment. 

 

Meetings

Meetings are bi-monthly (January, March, May, July, September, November) on the 2nd Tuesday of the month at 1:30pm. Contact us to join.

Meeting Materials

Open Meetings – All meetings are open to the public in accordance with the Maryland Open Meetings Act. Meeting documents shall be posted on alleganyspeaks.com in the Local Health Improvement Coalition (LHIC) action group. 

Community Health Needs Assessment  (CHNA)

  • Utilize local health data to identify areas of need

Community Health Improvement Plan (CHIP)

  • Develop a response to the CHNA by outlining priorities and creating an actionable plan

Implementation & Collaboration

  • Utilize the CHNA to develop Community Health Improvement Plan (CHIP)
  • Establish workgroups composed of local partners to address identified priorities
  • Coordinate efforts to ensure continued progress
